Gestalt Therapy Training Workshop: July 18 - 26, 2004

1. Basic, Advanced, Advanced Intensive Groups $ 1695.00
   ($800.00 room, board & fixed expenses + $895.00 tuition)
2. Clinical Practicum and Trainers Group $ 1895.00
   ($800.00 room, board & fixed expenses + $1095.00 tuition)

15% discount when attending both the Couples and Gestalt programs.

• Fixed expenses include brochure printing, postage, mailing and administrative costs
  in addition to shared accommodation (twin rooms with private bath) and all meals
  for the full 9 days.

Limited number of Single rooms - $200.00 supplemental charge for Couples Program, $350.00 for the Gestalt Program.

All moneys, from within the U.S., must be sent directly to GATLA in check or money order. Outside the USA, payments must be made in U.S. dollars only by an international money order or cashier’s check (not draft) from an international bank and drawn on a U.S. bank. Make checks payable to GATLA. Alternatively, fees may be paid by Visa, MasterCard, or American Express by mail or online through our secure server at www.gatla.org

CANCELLATIONS

Cancellations must be made in writing.

A $50.00 per person processing fee will be deducted from all refunds.
Full refund will be made for requests received by May 1, 2004.
50% refund will be granted for requests received by June 1, 2004.
No refunds will be issued for requests received after June 1, 2004.

SCHOLARSHIPS

A limited number of partial tuition scholarships are available up to a maximum of $600.00. To receive scholarship consideration please print and fax the completed Scholarship Application. NO SCHOLARSHIP WILL BE CONSIDERED WITHOUT COMPLETING THIS APPLICATION. This form must include the specific dollar amount of scholarship requested.  The scholarship application must be mailed or faxed due to security of personal data.
